Legal Notice

All license orders require a signature to certify the validity and authenticity of said license. All securities license orders are verified with FINRA. If you are a new securities licensee, the order will not be processed until verification of your license is posted online with FINRA broker check. Insurance licenses will be verified with State governing agencies when available online. When verification is not available online a hard copy of your license is required. Licensee name; lines of authority and license number must match. Any person willfully and knowingly misrepresenting themselves, deliberate deception, intentionally falsifying any information will be prosecuted to the fullest extent of the law.

It is your responsibility to know your company’s policy in regards to displaying any type of diploma certificates. Some companies prohibit the display of Law degrees, CPA diplomas, Professional Designation diplomas, College diplomas and any type of license certificates.

I certify that I have read this legal notice and agree to the terms and conditions as stated herein and indemnify and hold harmless SLD Industries. (its owners, officers, advisors, employees and agents as the same may be constituted now and from time to time hereafter) against any and all liability, losses, damages, product replacements and/or credits or any expenses whatsoever as a result of any claims, demands, damages, costs or judgments against it that may arise. I hereby waive and release SLD Industries Inc. from any and all liability.

Gift Waiver

If I am not the individual named on the certificate and/or license, I am purchasing this as a gift and have read and understand the terms and conditions of the legal notice, the verification process, my duty and responsibility to make sure it does not violate company policies and indemnify and hold harmless SLD Industries, Inc. as stated above. I hereby waive and release SLD Industries Inc. from any and all liability.
